City Star Assertive Community Treatment (ACT) provides 24-hour services and treatment to adults between the ages of 18 and 59 who have a serious mental illness. Clients have been homeless, or are at risk of homelessness, and may also have a co-occurring substance use disorder. Community-based services include therapeutic interventions, medication management and monitoring, comprehensive outpatient and community services, case management, employment support, peer support and supportive housing services.
